At the psycological level Ask yourself, what emotions could your unconscious mind be repressing such that your back pain becomes a significant attention diversion? In particular is there any anger or anxiety? These emotions could be buried deep in your subconscious and you may not be aware of any conflict consciously. At the physiological level Your unconscious mind creates a pain inducing diversion to keep these emotions repressed and this is not necessarily confined to the back.
